 Thank you for choosing NexDent Solutions Inc. (“NexDent Solutions”) as your platform to enhance communication and collaboration within the dental community.
By signing up for an account, you agree to the following terms of service and privacy policy. This consent form outlines your rights and responsibilities as a user of the NexDent Solutions Portal and NexDent Solutions Staffing Network.
1. Consent to Data Collection and Use
By signing up for and using the NexDent Solutions Portal and the NexDent Solutions Staffing Network, you consent to the collection, storage, and use of your personal and professional information, including but not limited to contact details, qualifications, certifications, and other relevant data. This information will be used solely for the purposes of facilitating communication, collaboration, and staffing solutions within the NexDent Solutions networks.
2. Consent to Communication
You consent to receive communications from NexDent Solutions and other users of the platform, including patients, dental specialists, and potential employers or employees. These communications may be in the form of emails, messages within the portal, phone calls, or other communication methods supported by the platform.
3. Data Security and Confidentiality
NexDent Solutions is committed to maintaining the confidentiality and security of your data. To ensure that your information is protected in accordance with the highest standards, NexDent Solutions partners with Knack to meet HIPAA compliance requirements, including end-to-end data encryption, logging and audits of data access and account logout after 15 minutes of inactivity. All information provided to NexDent Solutions will be stored securely, and will only be accessible to authorized users of the platform in compliance with applicable privacy laws and regulations.
4. Professional Conduct
By using the NexDent Solutions portal, you agree to conduct yourself professionally and ethically. This includes, but is not limited to, providing accurate and up-to-date information, respecting patient confidentiality, and adhering to professional standards in all communications and interactions on the platform.
5. Employment and Staffing
If you are using NexDent Staffing Solutions for employment or staffing purposes, you acknowledge that NexDent Solutions is not responsible for any employment decisions or arrangements made between you and other users. You agree to independently verify any employment opportunities, contracts, or agreements made through the platform.
6. Consent to Terms of Service and Privacy Policy
By signing this consent form, you acknowledge that you have read, understood, and agree to be bound by the NexDent Solutions Terms of Service and Privacy Policy, which govern your use of the platform. These documents are available for review on the NexDent Solutions website.
7. Right to Withdraw Consent
You have the right to withdraw your consent to the use of your data or to terminate your account at any time. To do so, please contact NexDent Solutions at info@nexdentsolutions.com. Please note that withdrawing consent may limit your ability to use certain features of the platform.
8. Amendments
NexDent Solutions reserves the right to amend this consent form, the Terms of Service, and the Privacy Policy at any time. You will be notified of any significant changes, and continued use of the platform will constitute acceptance of the amended terms. 

Employees, Temp and Job Seekers

As an employee or associate of a provider connected to the NexDent Solutions Inc. (“NexDent Solutions”) portal, you are being granted access to sensitive patient information as part of your professional responsibilities. This consent form outlines your responsibilities, obligations, and the terms under which you are authorized to access and use this information.

1. Acknowledgment of Responsibility

By signing this consent form, you acknowledge that you have been granted access to patient information solely for the purpose of fulfilling your professional duties within the dental practice or organization that employs you. You understand that this access is a privilege that comes with significant responsibilities, including maintaining the confidentiality and security of patient data.

2. Compliance with HIPAA and Privacy Regulations

You agree to comply with all applicable laws and regulations, including the Health Insurance Portability and Accountability Act (HIPAA), governing the privacy, security, and use of patient information. You acknowledge that NexDent Solutions, in partnership with Knack, has implemented stringent measures, including end-to-end data encryption, to ensure compliance with HIPAA and other privacy laws. As an authorized user, you are required to follow these protocols and any additional privacy policies established by your employer.

3. Confidentiality Agreement

You agree to maintain the confidentiality of all patient information to which you have access through the NexDent Solutions portal. This includes, but is not limited to, patient medical records, treatment plans, personal identification information, and any other sensitive data. You understand that unauthorized access, use, or disclosure of patient information is strictly prohibited and may result in disciplinary action, termination of employment, and/or legal consequences.

4. Access Limitation

Your access to patient information is limited to what is necessary to perform your job duties. You agree not to access, view, or share patient information beyond the scope of your responsibilities. You also agree to report any unauthorized access or breaches of patient data to your employer and NexDent Solutions immediately.

5. Use of the NexDent Solutions Portal

You agree to use the NexDent Solutions portal in a professional and ethical manner. This includes respecting the privacy of all patients, following all security protocols, and using the platform solely for its intended purpose. You acknowledge that your activities on the platform may be monitored to ensure compliance with these terms.

6. Data Security Obligations

You agree to take all necessary precautions to protect patient data from unauthorized access or disclosure. This includes safeguarding your login credentials, logging out of the portal when not in use, and following any additional security measures implemented by NexDent Solutions or your employer.

7. Consent to Terms of Use and Privacy Policy

By signing this consent form, you acknowledge that you have read, understood, and agree to be bound by the NexDent Solutions Terms of Use and Privacy Policy, as well as any additional policies or procedures established by your employer regarding the use of patient information.

8. Right to Terminate Access

Your access to the NexDent Solutions portal may be revoked at any time by your employer or NexDent Solutions if it is determined that you have violated any of the terms of this consent form, engaged in unauthorized use of patient information, or are no longer employed by the connected provider.
